If you wish to leverage the power of jot form to make a quick lead capture/pre-qual form, you can follow this quick how-to. The video is 15 mins in duration and shows how to set up your Jotform to post new leads to the system.
Note: The Jotform endpoint will be: https://app.lendingwise.com/jotform.php redirect=https://yourwebsite.com/thank-you
- You will need to change the "https://yourwebsite.com/thank-you" part to a valid url on your website.
- You will also need to set the arc and brc values in the form.

You can also import our template into Jotform by using the following directions (please remember you would still need to input your arc, fileType and brc fields) :
In jotform: Ceate a from -> Import Form -> From a web page -> Enter URL
Copy and paste the following url:

Endpoint/Submitter: https://app.lendingwise.com/jotform.php?redirect=https://www.lendingwise.com
After you import it, you can find the arc and brc values you need by following this screenshot (If you do not have access, please contact support@lendingwise.com): You need to put these arc and brc values into the Jotform. To get the arc and brc codes, answer the 4 dropdown questions and they will then be populated on the page.
